Anal Fistula Management: A Patient’s Experience with Dr. Preeti Paliwal

Patient Profile:

A 24-year-old male presented to Dr. Preeti Paliwal with complaints of a painful swelling and pus discharge in the anal region for the past 15 days.

The patient also reported occasional constipation for almost 10 years and a recent fissure surgery (fissurectomy with LIS) performed 10 days earlier at another hospital.

He had no history of systemic illness, drug or food allergies, and bowel movements were regular with medication. Appetite was reduced, but there were no urinary complaints.

Clinical Findings:

On examination, a tender mass was noted at the 6 o’clock position around the anal region.
A small external opening was visible approximately 1 cm below the anal verge, with pus discharge on pressure.
A cord-like tract was felt leading towards the anal canal, and sphincter tone was normal on digital rectal examination (DRE).

Provisional Diagnosis:

Anal Fistula (6 o’clock position) with active infection.

Treatment Approach by Dr. Preeti Paliwal:

Dr. Preeti Paliwal evaluated the condition thoroughly and advised complete management of the infection and fistulous tract.
The treatment plan included:

  • Sitz baths and dietary advice to improve hygiene and bowel movements
  • High-fiber diet and adequate hydration to prevent constipation
  • After infection control, definitive surgical management of the fistula was performed using a minimally invasive technique, ensuring preservation of sphincter function

Outcome:

The patient showed marked improvement in pain, discharge, and swelling within a few days after infection control.
Following the surgical procedure, recovery was smooth — with no recurrence and complete healing observed on follow-up.

If you are suffering from persistent anal pain, swelling, or discharge, expert care is available.
